In 1970, we reported how Apollo 14 astronauts Alan B. Shephard and Edgar D. Mitchell would have made it to the top of Cone Crater during their second exploration of the Moon if they had not been misled by a high ridge they mistook for the crater's rim. 

The terrain made a slight dip, which caused them to see the ridge as the crater rim, and the huge boulder field they were in made it extremely difficult to judge distances or perspective.
